Position: Lifecare Primary Care LPN

Location: Seneca Falls, NY

Pay: $1,342.80 per week

Key Responsibilities of the Primary Care LPN

• Coordinate patient flow from registration through discharge to minimize wait times and ensure efficient care delivery.
• Perform nursing assessments, obtain and document vital signs, and complete quality metrics in the EMR.
• Prepare patients for office visits by obtaining relevant provider notes and laboratory results.
• Assist providers with office-based procedures, examinations, and patient care.
• Schedule office visits and procedures, including obtaining prior authorizations.
• Assess patient understanding of health conditions and provide education and guidance.
• Manage patient phone calls and provide timely follow-up for issues.
• Coordinate prescription refill requests and monitor diagnostic results.
• Perform clinical testing and treatments as required, including phlebotomy and vaccine administration.
• Conduct CLIA-waived testing and maintain office supply inventory.
• Recognize and respond to medical emergencies.

Requirements of the Primary Care LPN

• NYS LPN license
• 1-year experience as an LPN in a physician's practice setting
• Pediatrics experience required
• BLS certification (AHA or RQI only)
