Key Responsibilities of the Licensed Practical Nurse (LPN)

• Collects data on residents' physical and psychosocial conditions and communicates findings to the RN and interdisciplinary team.
• Contributes to a culturally and age-appropriate care plan, involving residents and families in the process.
• Recommends revisions to care plans and identifies referral needs to address comprehensive resident care.
• Maintains clinical expertise through ongoing education and participates in mentoring staff.
• Provides leadership under RN supervision, ensuring safety and efficacy in care delivery.

Requirements of the Licensed Practical Nurse (LPN)

• NY LPN Licensure
• BLS certification (AHA or RQI)
• 1 year of experience as an LPN, preferably in a long-term care facility
