Job Title: LPN Part Time

Pay: $42.52 per hour

Shifts: 2/8s one week, 3/8s next week + Every Other Weekend and Holidays

Key Responsibilities of the LPN Part Time

• Collect data relevant to resident's physical and psychosocial condition and communicate findings to RN and the Interdisciplinary Team.
• Contribute to creating a culturally relevant care plan to achieve desired resident outcomes, involving the resident/family and healthcare team.
• Recommend care plan revisions based on evaluation of resident outcomes, collaborating with the team to ensure comprehensive care.
• Maintain current knowledge and competency in nursing practice, participating in mentoring/orientation of staff and care planning.
• Consider safety, cost, and impact factors when delivering care and providing leadership in the professional practice setting under RN direction.

Requirements of the LPN Part Time

• NY LPN Licensure
• Basic Life Support (BLS) certification from AHA or RQI
• 1 year experience serving as an LPN in a long-term care facility
