This role focuses on providing both direct and indirect skilled nursing care to adult and geriatric residents, under the supervision of a Registered Nurse. Key Responsibilities of the Licensed Practical Nurse (LPN)

• Collects data relevant to resident's physical and psychosocial condition and communicates findings to the RN and other members of the Interdisciplinary Team.
• Contributes to the development of age-appropriate and culturally relevant resident care plans.
• Recommends revisions to care plans based on evaluation of resident outcomes, ensuring comprehensive care.
• Maintains current knowledge and competency in nursing practice and participates in mentoring/orientation of staff.
• Considers factors related to safety, cost, and impact in the delivery of care.

Requirements of the Licensed Practical Nurse (LPN)

• NY LPN Licensure
• BLS (AHA or RQI required)
• 1 year of experience as an LPN in a long-term care facility
