Shifts: 7PM-7:30AM; Every other weekend and holiday

• Perform admissions and understand the admission process
• Coordinate transfers, discharges, and handle fall reporting
• Manage medication pass at the beginning of each shift
• Collaborate with CNAs to ensure quality care for residents
• Provide treatments and manage resident care plans during shifts

Requirements of the LPN/LVN - Long Term Care

• BLS Certification
• Valid LPN/LVN license
