Job Title: Licensed Practical Nurse (LPN)

Pay: $1,730.40 weekly

Shifts: 24 hours per 4 weeks, including 2 weekend shifts

Key Responsibilities of the Licensed Practical Nurse (LPN)

• Collects data on resident's physical and psychosocial condition and communicates findings to the RN and interdisciplinary team.
• Contributes to creating a culturally relevant care plan reflecting nursing interventions, promoting a healthy and safe environment.
• Recommends revisions to care plans and indicates need for referrals, ensuring comprehensive resident care.
• Maintains current nursing knowledge through continuing education and participates in staff mentoring/orientation.
• Considers safety, cost, and impact in care delivery and leads professional practice settings under RN direction.

Requirements of the Licensed Practical Nurse (LPN)

• Current NY LPN Licensure and BLS certification
• 1 year of experience as an LPN, preferably in a long-term care facility
