This full-time position requires weekend availability and involves working within a structured care environment, providing support to adult and geriatric patients. As an LPN, you will work under the supervision of a Registered Nurse to deliver both direct and indirect skilled nursing care to residents.

Key Responsibilities of the LPN Full Time + every weekend (40 hours)

• Collects data relevant to residents' physical and psychosocial conditions and communicates findings to the RN and Interdisciplinary Team.
• Contributes to the development of care plans that are age and culturally appropriate, involving residents and families.
• Revises care plans based on evaluations of resident outcomes and collaborates with the healthcare team to ensure comprehensive care.
• Maintains current knowledge and competency in Nursing Practice, participates in staff mentoring, and provides leadership under RN direction.
• Demonstrates knowledge and skills necessary for providing care to adult and geriatric residents.

Requirements of the LPN Full Time + every weekend (40 hours)

• NY LPN Licensure
• BLS Certification
• 1 year of experience as an LPN, preferably in a long-term care facility
