Location: Waterloo, NY

Pay: $1,700.80 weekly

Shifts: EOW and Holiday required

As a Licensed Practical Nurse, you will provide direct and indirect skilled nursing care to geriatric residents at a reputable facility. Key Responsibilities of the LPN - PT Nights

• Collects data relevant to resident's physical and psychosocial condition on an ongoing basis and communicates findings to RN and other members of the Interdisciplinary Team.
• Contributes to creating an age-appropriate and culturally relevant care plan, reflecting nursing diagnoses and interventions to achieve desired outcomes.
• Recommends care plan revisions after evaluation of outcomes and indicates the need for referrals, ensuring comprehensive resident care.
• Maintains current knowledge and competency in nursing practice, participating in mentoring and orientation of staff.

Requirements of the LPN - PT Nights

• NY LPN Licensure
• BLS certification through AHA/RQI
• 1 year experience as an LPN
• Experience in an LPN capacity at an LTC facility
