Job Title: LPN Part Time (2 Shifts/3 Shifts Rotational)

Location: Waterloo, NY

Pay: $42.52 per hour

Shifts: Weekend/Holiday required

This role involves providing direct and indirect skilled nursing care under the supervision of a Registered Nurse. Key Responsibilities of the LPN

• Collects data relevant to residents' physical and psychosocial conditions and communicates findings to RN and the Interdisciplinary Team.
• Contributes to the development of a culturally relevant plan of resident care with the healthcare team, promoting a healthy and safe environment.
• Recommends revisions to the care plan based on resident outcomes, ensuring all care aspects are addressed through collaboration.
• Maintains current knowledge and competency in Nursing Practice, participating in staff mentoring/orientation.

Requirements of the LPN

• NY LPN Licensure
• BLS certification through AHA or RQI
• 1 year of experience in an LPN capacity at an LTC Facility
