Seven Healthcare are seeking a Licensed Practical Nurse (LPN) for a full-time travel assignment in Waterloo, NY. This role involves day, evening, night, and weekend rotations with holiday shifts required. The successful candidate will demonstrate the necessary skills and knowledge to care for adult and geriatric patients, working collaboratively under RN supervision.

Key Responsibilities of the LPN

• Provide direct and indirect skilled nursing care for assigned residents, following policies and procedures.
• Collect data on resident's physical and psychosocial condition, communicating findings to RN and the Interdisciplinary Team.
• Contribute to the development of age-appropriate and culturally relevant care plans.
• Recommend revisions to care plans and ensure resident care needs are met.
• Maintain nursing practice knowledge and competency, participating in staff mentoring/orientation.

Requirements of the LPN

• NY LPN Licensure
• BLS (AHA or RQI only)
• 1 year experience as an LPN
• Experience serving in an LPN capacity at an LTC Facility
