Shifts: M-F 8-5 (No Weekend)

Key Responsibilities of the Physical Therapist

• Perform physical therapy evaluation and treatment of patients based upon physician orders/referrals.
• Complete comprehensive evaluations using appropriate evaluation and assessment tools and procedures.
• Review patient records for current diagnosis, past medical history, precautions, and contraindications.
• Establish measurable goals and develop treatment plans to promote maximum independence and function.
• Regularly reassess the effectiveness of treatment plans and patient progress toward goals.
• Communicate patient needs and progress to the treatment team, physician, and family members.
• Consult with medical providers regarding evaluation, treatment progress, and discharge.
• Recommend and assist in securing adaptive equipment to improve patient function.
• Maintain and submit documentation in compliance with regulations and company policies.
• Supervise Physical Therapist Assistants and Aides according to state practice acts.
• Attend and participate in staff development programs and obtain continuing education as required.

Requirements of the Physical Therapist

• Active PT AR License
• Experience with HCHB and OASIS
• Previous Home Health experience
