Pay: $2156.00 weekly

Shifts: Monday - Friday, 8am-5pm

This position involves providing physical therapy services based on physician orders in an onsite setting. The successful candidate will utilize their skills in physical therapy evaluation and treatment planning to support patient recovery and independence.

Key Responsibilities of the Physical Therapist

• Performs physical therapy evaluation and treatment of patients based upon physician orders/referrals.
• Completes a comprehensive evaluation using appropriate evaluation and assessment tools and procedures.
• Reviews patient records for current diagnosis, past medical history, precautions, and contraindications.
• Establishes measurable goals and develops treatment plan to move the patient toward maximum independence or function.
• Regularly re-assesses treatment plan effectiveness, patient progress, and modifies goals or treatment as needed.
• Communicates patient needs and progress to the treatment team, physician, and family members as applicable.
• Consults with medical providers regarding patient evaluation and discharge from treatment.
• Recommends and secures adaptive equipment for patient use to enhance rehab program.
• Maintains and submits required documentation in accordance with regulations.
• Supervises Physical Therapist Assistants and Aides in direct patient care and related activities.
• Attends internal staff development programs, maintains clinical practice skills.

Requirements of the Physical Therapist

• Current WV license
• Proficiency in HCHB
• Prior physical therapy experience
