Pay: $3,363.20 weekly

Shifts: M-F, 8-5 pm, no weekends

Key Responsibilities of the Physical Therapist

• Perform physical therapy evaluation and treatment based upon physician orders/referrals
• Complete comprehensive evaluations using appropriate tools and procedures
• Review patient records for current diagnosis and history
• Establish measurable goals and develop treatment plans for maximum independence
• Regularly re-assess treatment effectiveness and modify as needed
• Communicate patient needs and progress to the treatment team and family
• Consult with medical providers regarding evaluations and patient progress
• Recommend and assist in securing adaptive equipment for patients
• Maintain and submit required documentation
• Supervise Physical Therapist Assistants and Aides
• Participate in continuing education and staff development programs

Requirements of the Physical Therapist

• Active Physical Therapy license in the state of Pennsylvania
• Proficiency in Oasis and HCHB software
• Experience in a home health setting is highly desirable
