Key Responsibilities of the Physical Therapist

• Perform physical therapy evaluations and treatments based on physician orders/referrals
• Complete comprehensive evaluations using appropriate tools and procedures
• Review patient records for current diagnosis, medical history, precautions, and contraindications
• Establish measurable goals and develop treatment plans to enhance patient independence and function
• Regularly reassess treatment plans and modify as needed
• Communicate patient needs and progress to the treatment team and relevant parties
• Consult with medical providers regarding evaluations, treatments, and discharge planning
• Recommend and assist in securing adaptive equipment to improve function
• Maintain and submit documentation in line with Company policies and regulations
• Adhere to professional practice standards and legal regulations
• Supervise Physical Therapist Assistants and Aides in patient care activities
• Participate in and/or conduct staff development programs; maintain clinical practice skills

Requirements of the Physical Therapist

• Valid WV PT license
• Preferred experience with Home Health and OASIS, HCHB
