Key Responsibilities of the Physical Therapist

• Perform physical therapy evaluation and treatment based on physician orders/referrals
• Complete comprehensive evaluations using approved tools and procedures
• Review patient records for current diagnosis and medical history
• Establish measurable goals and develop a treatment plan for patient independence
• Re-evaluate treatment plans and patient progress, modifying as necessary
• Communicate patient progress with treatment team and involved parties
• Consult with medical providers about patient treatment and discharge
• Recommend and help secure special adaptive equipment for patients
• Maintain and submit required documentation in compliance with regulations
• Supervise Physical Therapist Assistants and Aides as per state practice act
• Participate in staff development programs and maintain clinical skills

Requirements of the Physical Therapist

• Active GA Physical Therapist License
• Experience in home health care
• Proficiency with HCHB (Home Care Home Base) preferred
