Key Responsibilities of the Physical Therapist

• Evaluate patient needs and develop individualized treatment programs
• Implement and administer complex therapeutic exercises
• Instruct and assist patients and caregivers in improving functional mobility
• Provide clinical supervision of Physical Therapist Assistants and Home Health Aides
• Ensure appropriate use of adaptive equipment
• Maintain effective communication with the interdisciplinary care team

Requirements of the Physical Therapist

• CO License or compact license accepted
• Experience with HCHB and Home Health
• OASIS experience required
