Job Title: Physical Therapist

Location: Finger Lakes Region, NY

Pay: $2387.20 weekly

Shifts: Monday - Friday 8-4:30pm; weekends as needed

Seven Healthcare are seeking a skilled Physical Therapist to join the Finger Lakes team, specializing in home health. This is a travel/contract position where you will provide case management and direct patient care in a supportive team environment.

Key Responsibilities of the Physical Therapist

• Provide case management and direct patient care
• Administer physical therapy modalities in a home setting
• Assist the physician in evaluating level of function and develop plan of care
• Prepare clinical and progress notes
• Consult with families and agency personnel
• Participate in in-service programs

Requirements of the Physical Therapist

• B.S., M.S. or D.P.T. degree in Physical Therapy
• Possession of a current New York State License in Physical Therapy
• BLS (AHA ONLY)
• Minimum of one year experience in a Long Term Care/Home Care setting as a Physical Therapist
• Recent Home Health Experience within the last year
• Oasis experience is required
