Location: New York, NY

Pay: $2,387.20 weekly

Shifts: Monday - Friday 8-4:30 pm; weekends as needed

Seven Healthcare is seeking a skilled Physical Therapist to join a dedicated team in New York as part of a travel assignment. The successful candidate will provide case management and direct patient care within a home setting, supporting the Monroe County team exclusively. Key Responsibilities of the Physical Therapist

• Provide comprehensive physical therapy modalities in the home setting
• Assist physicians in evaluating patient levels of function and developing care plans
• Prepare clinical and progress notes and participate in in-service programs
• Consult and educate patients and families about disease processes and preventive strategies
• Ensure compliance with state and federal regulations through appropriate documentation
• Maintain good working relationships with patients, families, and colleagues

Requirements of the Physical Therapist

• B.S., M.S., or D.P.T. degree in Physical Therapy
• Current New York State License in Physical Therapy
• BLS certification (AHA ONLY)
• Minimum of one year experience in Long Term Care/Home Care setting
• Recent Home Care experience within the last year
• Strong verbal and written communication skills
