Key Responsibilities of the Physical Therapist

• Perform physical therapy evaluation and treatment based on physician orders/referrals.
• Complete comprehensive evaluations using appropriate tools and procedures.
• Review patient records for diagnosis, medical history, and precautions.
• Develop treatment plans with measurable goals for patient independence and function.
• Re-assess treatment effectiveness and modify plans as necessary.
• Communicate patient progress to treatment team and medical providers.
• Recommend and assist in provisioning adaptive equipment to enhance rehabilitation.
• Maintain and submit required documentation as per policy and regulations.
• Supervise Physical Therapist Assistants and Aides in patient care activities.
• Participate in staff development, meetings, and obtain continuing education as required.

Requirements of the Physical Therapist

• Active NC PT license
• Experience with Home Health PT, Oasis, and HCHB
