Job Title: RN - Case Management - Travel

Location: Covington, GA

Pay: $2,599.20 weekly

Shifts: M-F 8am-5pm, On Call Schedule: Yes, 7 days once a month

Key Responsibilities of the RN - Case Management - Travel

• Perform comprehensive assessment of high-risk members.
• Collaborate with primary care providers to implement individual care plans.
• Address and reassess cost-effective, appropriate levels of care.
• Coordinate non-medical support to ensure treatment compliance.
• Coordinate care transitions with healthcare team.
• Engage specialty resources for optimal patient outcomes.
• Maintain detailed records of clinical and functional outcomes.
• Identify opportunities for health promotion and illness prevention.
• Act as patient advocate protecting privacy and confidentiality.

Requirements of the RN - Case Management - Travel

• Current, unrestricted GA License or compact licenses.
• Associates or Bachelor's Degree in Nursing or related field.
• Case Management certification.
• Experience with HCHB, OASIS, and home health.
• Ability to analyze complex medical information and make rational decisions.
