Shifts: 8A - 4:30P with weekend rotation

• Coordinate patient discharge planning activities
• Utilize mobile and computer equipment effectively

Requirements of the RN - Case Manager

• Minimum 1-year current Case Management RN experience
• Experience in discharge planning and emergency department
• Knowledge in palliative care and GOC conversation
