Job Title: RN Case Manager

Location: Fort Pierce, FL

Pay: $2,232 per week

Shifts: 8-hour shifts, rotating every other weekend from 8 am - 4:30 pm

Seven Healthcare are seeking an experienced RN Case Manager for an exciting travel assignment in Fort Pierce, Florida. This role requires adaptability across various units of a fast-paced, high-acuity level II trauma hospital environment. Candidates will initially orient at Palms West before transitioning to their assignment.

Key Responsibilities of the RN Case Manager

• Participate in RN/SW/CMA triad model for comprehensive case management
• Conduct discharge assessments using Interqual for Initial and CSR
• Work collaboratively across units in a fast-paced environment

Requirements of the RN Case Manager

• Active RN Florida/Compact License
• 2-4 years of Case Management experience in an acute care facility
• Familiarity with Meditech, Midas, Nate, Outlook, and Imobile
• Ability to adapt to various unit assignments and float as required
