Job Title: RN - Case Management - Travel

Location: Gainesville, GA

Pay: $2,546.40 weekly

Shifts: M - F 8am to 5pm (On-call 1 weekend/month and occasional after-hours call during the week)

• Perform comprehensive assessment of high-risk members
• Collaborate with primary care providers to implement individualized care plans
• Reassess levels of care for cost-efficiency and appropriateness
• Coordinate non-medical support for compliance with treatment plans

Requirements of the RN - Case Management - Travel

• Associates or Bachelor's Degree in Nursing or related field
• Current, unrestricted state Registered Nurse license

• Case Management certification
