• Screen new admissions daily for discharge planning needs
• Verify accuracy of demographics and payer information
• Coordinate patient appeal rights under the Discharge Appeals program
• Facilitate third-party reimbursement through effective communication
• Ensure discharge plans match patient needs and covered services
• Advocate for patients regarding insurance coverage and care requirements
• Maintain comprehensive knowledge of community resources

Requirements of the RN - Case Management

• Current registration as a Registered Nurse in the Commonwealth of Massachusetts
• Minimum of 3 years acute care experience within the past 5 years
• Working knowledge of InterQual or equivalent system
• Demonstrate experience in Discharge Planning and Utilization Review
• Ability to work independently in a fast-paced environment
