Job Title: RN - Case Management - Travel

Location: Macon, GA

Pay: $2,546.40 weekly

Shifts: Monday - Friday, 8 am to 5 pm, on call once every three weeks

This role focuses on providing high-quality home health patient care, utilizing your expertise in HCHB/OASIS, and requires strong collaborative skills to ensure the delivery of comprehensive care plans.

Key Responsibilities of the RN - Case Management - Travel

• Perform comprehensive assessment of high-risk members
• Collaborate with primary care providers for individualized care plans
• Address and reassess appropriate levels of care efficiently
• Implement non-medical support for treatment compliance
• Coordinate care transitions and liaise with healthcare professionals
• Engage specialty resources for optimal resolution
• Maintain detailed records of clinical and fiscal outcomes
• Identify opportunities for health promotion and illness prevention
• Act as a patient advocate with privacy and confidentiality considerations

Requirements of the RN - Case Management - Travel

• MUST HAVE HCHB/OASIS and home health experience
• GA RN License, willing to accept compact license
• Associates or Bachelor's Degree in Nursing or related field
• Current, unrestricted state Registered Nurse license
• Case Management certification
• Ability to analyze complex medical information
• Experience with wound and IV care
