Job Title: Level 2 Registered Nurse Case Manager

Location: NY

Pay: $2,834.80 per week

Shifts: Monday - Friday, Day shift 8:00am-4:30pm, weekends per need

Seven Healthcare are seeking a dedicated Level 2 Registered Nurse Case Manager to join a dynamic care team in NY. This position involves providing skilled nursing care and comprehensive case management in a home health care setting, ensuring an optimal work-life balance with scheduled daytime shifts.

Key Responsibilities of the Level 2 Registered Nurse Case Manager

• Develop and revise treatment and discharge plans with the care team
• Complete and submit OASIS assessments and related documentation
• Process medical orders and coordinate with physicians on patient care
• Educate patients and families on disease processes and self-care
• Supervise LPNs to ensure patient health and compliance with care plans
• Coordinate and evaluate quality of patient care services
• Participate in agency operational and financial objectives
• Contribute to professional growth and respect for all employees

Requirements of the Level 2 Registered Nurse Case Manager

• Certified Home Health Care Experience Required
• EPIC or Netsmart experience preferred
• OASIS experience
• Telemetry experience
• Current RN License (NY License ONLY)
• One year of experience as a clinical nurse in a hospital required
• Critical thinking skills and ability to make clinical judgments
• Strong communication and assessment skills
• Current NYS driver’s license
• Basic Life Support (BLS) certification
