Job Title: Registered Nurse - Home Health

Location: San Antonio, TX

Pay: $2,208.40 per week

Shifts: M-F 8am-5pm (On call, once a month)

Seven Healthcare are seeking an experienced Registered Nurse specializing in Home Health for a contract assignment in San Antonio, TX. This role involves working closely with patients in the community, ensuring health maintenance, disease prevention, and case management.

Key Responsibilities of the Registered Nurse - Home Health

• Observe, assess, plan, implement, and evaluate nursing support for individuals across health spectrums in a community or facility setting
• Collaborate with healthcare professionals to address health needs
• Coordinate communications and actions related to health and safety
• Manage medication processes including ordering, monitoring, and disposal
• Review and update policies related to medication management annually
• Communicate client condition changes to physicians
• Transcribe physician orders and complete necessary documentation
• Document health status in nurse's notes routinely
• Maintain guidelines to prevent disease spread through proper equipment care
• Supervise activities of licensed practical/vocational nurses
• Ensure health-related appointments and follow-ups are conducted timely
• Liaise with inpatient nursing units when patients are hospitalized
• Provide training for new employees and recertification as needed
• Train unlicensed staff on specific health, safety, and prevention needs

Requirements of the Registered Nurse - Home Health

• TX RN license or Compact License
• Experience with HCHB, OASIS, and wound care preferred
• Skilled in disease teaching, Pluerx Drain, JP Drain, Woundvac, IV antibiotics, PICC line, Ostomy, and Foley care preferred
