Pay: $2,752.00 per week

Shifts: Day shift Monday to Friday

Key Responsibilities of the RN - Case Management

• Perform comprehensive assessment of high-risk members
• Collaborate with primary care providers for individualized care plans
• Assess cost-efficient levels of care and put non-medical support in place
• Coordinate care transitions and engage specialty resources
• Maintain detailed records of outcomes during the management process
• Identify opportunities for health promotion and illness prevention
• Act as patient advocate by protecting privacy and confidentiality

Requirements of the RN - Case Management

• Associates or Bachelor’s Degree in Nursing or related field
• Current, unrestricted WV or compact Registered Nurse license
• Ability to analyze complex medical information and make informed decisions
• Case Management certification
