Home Health RN Antigo, WI

Assignment & Schedule

• Length: 13 weeks.
• Hours: 37 guaranteed weekly, with flexibility up to 40.
• Shift: Varied 8-hour shifts.
• Weekends: Every third weekend.
• Holidays: Rotating coverage.
• Call: 1-2 times during the week and every other weekend, per listing.
• Response time: Return answering-service calls within 15 minutes; triage by phone and make visits when clinically necessary.

Required Qualifications

• 2 years of home health experience and prior travel experience required.
• Active Wisconsin RN license, BLS, and valid driver's license.
• Certifications must be AHA or American Red Cross.
• OASIS experience required; OASIS certification preferred.
• Experience with Epic Remote Client/Remote Care.

Caseload & Travel

• Approximately 20-25 patients, averaging 4.5 visits/day.
• Approximately 50 driving miles/day, depending on territory and census.
• May float to other territories during low census.

Clinical Expectations

• OASIS admissions and all assessment time points.
• Episode management, provider communication, care coordination, and timely documentation.
• Wound care, IV therapy, port care, lab draws, and chronic disease management.

Facility Policies

• Listed 50-mile radius restriction includes an expiration note of 10/23; confirm current applicability.
• Hours are not guaranteed during orientation week or holidays when the unit is closed.
• Contract permits 36 hours of non-billable low census time; low census on-call hours count toward this allowance.
• Callback pay applies when called back, with a 2-hour minimum.
• During flu season, travelers declining flu/COVID vaccination must wear a mask at all times, per facility policy.
• Interview: Manager phone interview, Monday-Friday.
