Job Description & Requirements

Home Health RN Monday-Friday

• Schedule: 8:00 a.m.-4:00 p.m.
• Provide skilled nursing care to patients of all ages in their homes.
• Assess physical, psychosocial, environmental, and pain-management needs, including stable and emergent situations.
• Develop, implement, and evaluate individualized plans of care.
• Coordinate services with patients, families, physicians, referring agencies, and the interdisciplinary team.
• Educate patients and caregivers on medications, diet, home safety, infection prevention, and self-care.
• Maintain timely, accurate documentation and monitor payer requirements.
• Supervise and provide clinical guidance to home health aides, STNAs, and LVNs.

• Arrange referrals and participate in interdisciplinary care meetings.
